General
Information-Clinic Services
Clinic
services are provided by
the school nurse and nurse
assistant in the event of
illness or injury occurring
during the school day. A
student may go to the health
room (clinic) only after
obtaining a corridor pass
from his/her classroom teacher.
Failure to report to the
health room (clinic) after
leaving class with a corridor
pass is considered truancy.
Students who leave school
without going through the
clinic and office will be
considered truant.

Cards
Each
student is issued information
and/or clinic cards which
the parents must complete,
sign and return. The information
on these cards is of vital
importance in giving the
administrators and health
services staff directions
for proper care of the student
in case of injury, illness
or other emergencies. Accurate
completion of these cards
is an enrollment requirement
of the district. It is of
utmost importance that parents
contact the school office
to designate an adult for
supervision when they must
be out of town.
