Immunization Requirements
for Kindergarten - 12th Grade Students
Diphtheria/Pertussis/Tetanus - At age 5 or upon entry into kindergarten,
five doses of vaccine in any combination are required unless the fourth
dose was received on or after the fourth birthday, in which case only
four doses are required. Students are 7 or older are required to have
at least three doses of a tetanus-diphtheria containing vaccine. One
dose of tetanus-diphtheria containing vaccine is required within the
last 10 years.
Polio - Upon entry into kindergarten, four doses are required, one
of which must have been received on or after the fourth birthday. If
the third dose was administered on or after the fourth birthday, only
three doses are required. If any combination of four doses of OPV and
IPV was received before four years of age, no additional dose is required.
(Polio vaccine is not required for students 18 years old or older.)
Measles* (Rubeola) - Two doses of measles-containing vaccine are required.
The first dose shall be administered on or after the first birthday
with the second dose due by age 5, or upon entry into kindergarten.
Rubella*(German Measles) and Mumps - One dose each of rubella and
one dose of mumps vaccine received on or after the first birthday is
required. (May be given in MMR combination vaccine.)
Hepatitis B* - Three doses are required for the following school years:
(a) 2005-2006 for kindergarten through 11th grade; and (b) Beginning
in school year 2006-2007, for all students in kindergarten through
12th grade. (Two dose RECOMBIVAX is acceptable for adolescents 11 to
15 years old.
Varicella* (Chickenpox) - One dose of varicella vaccine received on
or after the first birthday is required for the following grades for
the following school years: (a) 2005-2006 for kindergarten through
fifth grade and seventh through 11th grade; and (b) Beginning in school
year 2006-2007, for all students in kindergarten through 12th grade.
Two doses are required if the child was 13 years old or older at the
time the first dose of varicella vaccine was received. Reliable parent
history or physician validation of varicella illness may meet requirement.
- ·Serologic confirmation of illness will meet requirement
for Measles, Rubella, Mumps, Varicella and Hepatitis B. Vaccine doses
administered less than or equal to four days before the minimum interval
or age shall count as valid.
A different schedule and dose series number may apply for children
who have delayed immunizations. (Please see school nurse for catch-up
schedules and minimum dose intervals. There is no need to restart a
series regardless of elapsed time between doses.)
