Immunization
Requirements- Children Less than Five Years of Age
Diphtheria/Pertussis/Tetanus (DPT/DTaP)
- Children less than two months old: no vaccine is
required.
- Children two months of age, but not yet four months
of age: one dose is required.
- Children four months of age, but not yet six months
of age: two doses are required.
- Children six months of age, but not yet eighteen
months of age: three doses are required.
- Children eighteen months of age, but not yet five
years of age: four doses are required.
Polio
- Children less than two months old: no vaccine is
required.
- Children two months of age, but not yet four months
of age: one dose is required.
- Children four months of age, but not yet six months
of age: two doses are required.
- Children six months of age, but not yet eighteen
months of age: three doses are required.
- Children eighteen months of age, but not yet five
years of age: three doses are required.
Measles (Rubeola)
- Children from birth to twelve months of age: no
vaccine is required.
- Children twelve months of age, but not yet five
years of age: one dose of vaccine is required on or after the first
birthday or serologic confirmation of measles illness.
Rubella (German Measles)
- Children from birth to eighteen months of age:
no vaccine is required.
- Children eighteen months of age, but not yet five
years of age: one dose of vaccine is required on or after the first
birthday or serologic confirmation of rubella illness.
Mumps
- Children from birth to eighteen months of age:
no vaccine is required.
- Children eighteen months of age, but not yet five
years of age: one dose of vaccine on or after the first birthday is
required or serologic confirmation of mumps illness
Haemophilus Influenzae (Type b conjugate vaccine
(HibCV)
- Children less than two months of age: no vaccine
is required.
- Children two months of age, but not yet four months
of age: one dose of vaccine is required.
- Children four months of age, but not yet fifteen
months of age: two doses of vaccine are required.
- Children fifteen months of age, but not yet five
years of age: one dose on or after fifteen months of age is required
unless a schedule for a primary series and booster was met prior to
or at fifteen months of age. (History of Hib disease since age two through
age four is accepted with provision of a written statement from a physician
licensed to practice medicine in the United States.)
Varicella (Chickenpox)
- Effective August 1, 2000, children born on or after
September 2, 1994, will be required to show proof of either one dose
of varicella vaccine received on or after the first birthday or a parent-
or physician-validated history of varicella illness or serologic confirmation
of varicella immunity.
Tuberculin Skin Test (TB)
- Evidence of a negative skin test having been done
in the United States since nine months of age and within the last twelve
months is required. A positive skin test will require evidence of negative
chest film. BCG immunization is not accepted.
All enrolling students must meet all immunization
requirements before being permitted to enroll.
The parent or legal guardian is responsible for providing
all immunization documents for the enrolling student before enrollment
can be completed.
Acceptable immunization documents are limited to official
health records from the previous school(s) attended and/or records verified
by a licensed physician or public health personnel.
